Gait speed in Parkinson disease correlates with cholinergic degeneration.

چکیده
Editors’ Note: Grabenhenrich and Roll critique the methodology and design in the study by Kerti et al. on the association between glucose metabolism and memory performance. The authors conduct additional statistical analysis to support their results. Neurology’s sometime neuromythologist and historian, William Landau, cites quotations from Parkinson’s historical monograph to comment on the article by Bohnen et al. on gait speed in Parkinson disease. The authors respond. —Chafic Karam, MD, and Robert C. Griggs, MD

Cholinergic mesencephalic neurons are involved in gait and postural disorders in Parkinson disease.
Gait disorders and postural instability, which are commonly observed in elderly patients with Parkinson disease (PD), respond poorly to dopaminergic agents used to treat other parkinsonian symptoms. The brain structures underlying gait disorders and falls in PD and aging remain to be characterized. Using functional MRI in healthy human subjects, we have shown here that activity of the mesenceph...
متن کاملAttentional Control of Gait and Falls: Is Cholinergic Dysfunction a Common Substrate in the Elderly and Parkinson’s Disease?
The aim of this study was to address whether deficits in the central cholinergic activity may contribute to the increased difficulty to allocate attention during gait in the elderly with heightened risk of falls. We recruited 50 participants with a history of two or more falls (33 patients with Parkinson's Disease and 17 older adults) and 14 non-fallers age-matched adults. Cholinergic activity ...
متن کاملAcetylcholinesterase inhibitors and gait: a steadying hand?
Cognitive impairment, complex movement disorders, and high risk of falls are all interrelated , levodopa-unresponsive symptoms in patients with advanced Parkinson's disease. Although Parkinson's disease is conventionally viewed as a motor syndrome that results from nigrostriatal dopaminergic denervation, there is also an intricate interplay between multisystem degeneration and neurotransmitter ...
متن کاملClinimetrics of the Freezing of Gait Questionnaire for Parkinson Disease During the “off” State
Introduction: Freezing of gait, a common PD motor symptom, could increase the risk of falling. This study aimed to investigate the clinimetric attributes of the Freezing of Gait Questionnaire (FOGQ) for people with Parkinson disease in the “off” state. Methods: A total of 115 patients with Parkinson disease (PD; mean age, 60.25 years) were included. Acceptability, internal consistency (by the ...
متن کاملCueing Effect of Gait Ability in Parkinson’s Disease
The idiopathic Parkinson’s disease (PD) results from a degeneration of dopamine-producing cells in the substantianigra which leads to cardinal symptoms of hypokinesia, bradykinesia, postural instability, rigidity and tremor. These symptoms lead to difficulties in motor performance such as gait ability and falls. Even optimal medication therapy, gait problems associated with PD are often charact...
